New Practice Guideline Co-Led by Penn Dental Medicine Details Dental Pain Management Strategies

A new clinical practice guideline for managing acute dental pain in adolescents and adults has been released, recommending nonsteroidal anti-inflammatory drugs (NSAIDs) taken alone or with acetaminophen as first-line treatments for managing short-term dental pain. The guideline was developed by Penn Dental Medicine's Center for Integrative Global Oral Health (CIGOH), the American Dental Association (ADA), and the University of Pittsburgh School of Dental Medicine. Dr. Alonso Carrasco-Labra, Associate Professor and Director of the Cochrane Oral Health Collaborating Center at Penn Dental Medicine, and Olivia Urquhart, an epidemiologist and instructor within CIGOH, co-led its development. Published in the February 2024 issue of JADA, the guideline has been endorsed by the ADA.

Based on a review of the best available evidence, the guideline panel concluded that, when used as directed, NSAIDs like ibuprofen and naproxen alone or in combination with acetaminophen can effectively manage pain after having a tooth removed or when experiencing a toothache when dental care is not immediately available.

"The recommended approach is to use nonopioid analgesics as the primary treatment instead of opioid analgesics," says Carrasco-Labra. "This is due to their effectiveness and lower risk profile."

The guideline also offers clinicians recommendations for prescribing opioid medications in the limited circumstances in which they may be appropriate. These include avoiding "just in case" prescriptions, engaging patients in shared decision-making, and exerting extreme caution when prescribing opioids to adolescents and young adults. When prescribing opioids, the guideline suggests advising patients on proper storage and disposal and considering any risk factors for opioid misuse and serious adverse events.

"Clinicians are encouraged to have conversations with their patients where they inform them to anticipate some level of discomfort and explain what it will feel like," adds Carrasco-Labra. "The prescribed analgesics should make the pain manageable but also align with what the patient seeks in terms of associated risks. If they're coming in for a procedure that will likely result in some mild discomfort, they need to be in the driver's seat in determining the appropriate pain-management strategy."

This is the second of two guidelines on acute dental pain management from this research team. A previous set of recommendations for pediatric patients was published in 2023. Both guidelines are can be found at ada.org/painmanagement.

The City of Hialeah, Smile Generation, and KABOOM! Unveil Kid-Designed Playground at Babcock Park

Community joins forces with nonprofit to advance playspace equity at Babcock Park


Kids at Babcock Park in Hialeah, Florida are celebrating as the city debuts a kid-designed, community-built playground. The months-long collaboration between The City of Hialeah, Smile Generation, and KABOOM! culminated this week as more than 200 volunteers built the new playspace that will spark joy and foster a sense of belonging for over 1,000 kids each year in Hialeah.

The new playspace with The City of Hialeah and Smile Generation is part of a nationwide effort led by KABOOM! to end playspace inequity – the reality that far too many children do not get the chance to play on a playground, something that should be part of every child’s life. Community members led the creation of the playground, from design to installation. In January, kids from Hialeah shared drawings of their dream playspaces with project organizers. These ideas were integrated into the new playground design.

“The City of Hialeah is so grateful for being selected by our friends at KABOOM! to receive nearly 100% funding for a new playground at Babcock Park,” said William Sanchez, Hialeah Parks and Recreation Director. “With the amazing contribution both in funding and labor from our corporate sponsors, Smile Generation, we were able to build an entire playground from scratch in one day! As part of the project, three organizations planned a special ‘Design Day’ for all the kids in the community. During Design Day, children were encouraged to put their imaginations on paper and design their ideal playground. With their input carefully considered, roughly 200 volunteers from our sponsors and the community met on Saturday, March 9th at Babcock Park to work cohesively to ultimately replace what was once an outdated and unsightly play space with a new, more interactive playground for the children in our community. Babcock Park is the City’s oldest recreational facility and its prime location near three primary schools and the City’s affiliation with the Miami Marlins Youth Academy reinforces the need and importance of this wonderful project. The park is open seven days a week and is utilized heavily by surrounding residents on a daily basis. A huge thank you to KABOOM!, Smile Generation, and those who worked tirelessly to bring the visions of the youth in our city to fruition.”

Playing is the essence of childhood and is a critical developmental need that provides countless physical, social, and mental health benefits. However, far too many children, especially in communities of color, do not have a place to play near where they live or learn. The new playground at Babcock Park, designed with the neighborhood’s unique needs and aspirations in mind, brings Hialeah one step closer toward ending playspace inequity and ensuring that no matter where a child lives or their background, they have limitless opportunities and joyful childhoods that are centered around play.

Smile Generation Ambassador Daris Hondoy was particularly excited to attend the unveiling of this playground because of his personal connection to the park.

Early childhood is the most important time to prevent oral diseases, which are correlated with serious long-term health risks, such as cancer and diabetes. Overjet's artificial intelligence provides dentists with the first objective standard that quantifies pathologies like tooth decay, down to the millimeter. • The primary clearance is for detecting pediatric caries — commonly known as cavities or tooth decay — in patients ages 4 and above, covering both primary and permanent teeth. Overjet's technology is unique in that it is cleared to precisely outline and quantify instances of caries, allowing dentists to monitor changes over time.

• A second clearance, called Charting Assist, expands the scope of Overjet for Kids by enabling the AI to detect dental structures in patients ages 5 and above. Benevis Report Emphasizes the Need to Prioritize Adolescent Dental Health in Honor of World Oral Health Day

Despite numerous advances in healthcare, achieving equitable access to dental treatment is still out of reach for one in three U.S. adolescents who do not have access to preventive dental care, according to the National Institute of Dental and Craniofacial Research.

Benevis, a leading dental healthcare and orthodontics delivery organization committed to providing quality care to underserved communities, announces the release of its report, The Impact of Neglected Adolescent Dental Care on Adult Health. In support of World Oral Health Day this March 20, 2024, the paper highlights pertinent research and data on the continuing need for clinical providers and families to reinforce positive dental health habits and orthodontic care from the ages of 12 to 19 years old to influence oral health into adulthood to critically impact overall health and well-being across all patient populations.

Many oral diseases and harmful health behaviors managed by adults begin in adolescence. About 57% of adolescents have had tooth decay in their permanent teeth, a preventable disease. Not only does it affect permanent teeth from early childhood into adolescence, but decay also negatively impacts the ability to eat and speak, which is critical to physical and psychosocial health. Older adolescents ages 16 to 19 are more likely to experience cavities, and those from lower-income households are twice as likely to have untreated decay as those from higher-income families. Harmful behaviors with implications for adverse oral health outcomes, like dietary habits, smoking, vaping, and substance use, also become more common in adolescent years.

In the U.S., social determinants of health (SDOH) have risk factors that are directly linked to poor oral health outcomes and are attributed to social, economic, and geographic barriers. Research has proven that 22% of adolescents from low-income households are twice as likely to have untreated decay as those not from low-income households. The same report found that non-Hispanic Black (20%) and Mexican American (21%) adolescents experience more untreated tooth decay than non-Hispanic White (16%) adolescents. Expanding access to equitable oral healthcare to support dental disparities during this vital stage of life is essential to adolescent health and well-being.

"Access to quality dental care should be easy, especially when it's essential; however, millions of adolescents face barriers to treatment or lack of dental hygiene resources and education for healthy smiles," said Bryan Carey, CEO of Benevis. "As adolescents move closer to adulthood or further away from the support of their parents' dental insurance coverage, establishing strong oral health habits that they can maintain over a lifetime becomes even more important. This work begins in the household and within their dental home--the ongoing relationship with a dental provider--to prevent the early onset of dental disease and teach healthy behaviors."

Dr. Grant Zakhar, Orthodontist at Youth Dentistry Georgia, a Benevis-managed practice with three offices in the state, shared that while some patients are interested in braces for cosmetic reasons, many have a real clinical need. “Some patients have so much crowding, their teeth can't come in, there's impacted teeth, or some growth of the jaw is needed to make room increasing the risk of places where plaque can stick that are hard to reach with a toothbrush and floss, leading to cavities,” he explained. “As an Orthodontist, I'm teaching the parents and the patient what's going on, what we do, and why it's not just putting colors on teeth. The education piece is essential to ensure for a healthy smile.”

CareQuest Institute Requests Grant Proposals Addressing Oral Health Disparities

Proposals Now Open for Veterans and School-Related Initiatives to Improve Oral Health Access and Outcomes

CareQuest Institute for Oral Health, a leading national nonprofit focused on creating a more accessible, equitable, and integrated oral health system, has launched two grant opportunities aimed at addressing systemic barriers to oral health access. The Requests for Proposals (RFPs) include: (1) School-Based and School-Linked Initiatives to Improve Oral Health Grant as well as (2) Veterans Oral Health Access and Equity Grant. Proposals are now open and are due by May 1, 2024. Ten projects, funded up to $125,000, will be selected for each RFP.

“In our pursuit of a more accessible and equitable oral health system, CareQuest Institute invites proposals aimed at breaking down barriers for our nation’s veterans and strengthening our school-based oral health programs,” said Trenae Simpson, director of grants and programs at CareQuest Institute. “Through these initiatives, we hope to authentically engage and amplify community voices, encourage innovation, and improve health outcomes for those who have often been unheard and left behind in our health and oral health care system.”

School-Based and School-Linked Initiatives to Improve Oral Health Grant

In alignment with its mission to enhance oral health outcomes, CareQuest Institute is inviting proposals for initiatives focused on school-based and school-linked interventions.

Schools can become the center for multiple opportunities to improve the oral health and overall health of the students, increasing their chances to stay in school and succeed in life. School-based and school-linked oral health programs have potential to improve health for children, reducing racial, ethnic, and economic disparities.

Relevant responses to the RFPs should reflect the applicant’s commitment to meaningfully and consistently engage members of the community throughout their proposed project. This definition of engagement goes beyond clinical treatment to nurture environments ripe for community empowerment and justice.

Examples include, but are not limited to:

· Workforce Considerations: Exploring effective student-facing roles (e.g., school nurses) and strategies individuals in these roles use to support the expansion of oral health prevention, messaging, and access to care for students.

· Community Engagement: Advocacy by community members to support policy to implement or expand school-based or school-linked oral health programs.

· Policy Solutions to Improve Oral Health: Local education agency, state, or national policies to support school-based or school-linked oral health programs.

· Expansion of the Capacity of Federally Qualified Health Centers (FQHCs): School-based oral health programs may either be operated by or partner with FQHCs to refer students and their family members, so that everyone eventually secures a dental home.

· School-Based Technology and Tools: Teledentistry can enable children to receive diagnostic, preventive, and early intervention dental care at their school.

· Alternative Payment Models: A clinical organization piloting innovative clinical, workforce, and workflow solutions to inform and/or validate creative payment models that support school-based or school-linked oral health.

Veterans Oral Health Access and Equity Grant

Recognizing the unique oral health challenges facing veterans, CareQuest Institute is also requesting proposals aimed at improving access and care for the 17 million veterans in the United States. Veterans are disproportionately affected by tooth decay and periodontal issues, making this grant opportunity critical in advancing oral health equity among this population.

Examples include, but are not limited to:

· A national organization developing a veteran-focused oral health policy agenda in collaboration with veteran self-advocates and veteran-focused partners.

· A state-based organization hosting community listening sessions and advocacy trainings to strengthen the capacity for oral health advocacy toward expanding access to care.

· A clinical organization piloting innovative clinical, workforce, and workflow solutions to inform and/or validate creative payment models that support oral health care for veterans.

· A community-driven approach where project design and decision-making is led by the veteran community.

Kenvue Presents New Clinical Evidence Showing Improved Protection Against Plaque and Gingivitis at the International Association for Dental Research

Kenvue Inc. (NYSE: KVUE) (“Kenvue”), the world’s largest pure-play consumer health company by revenue, will present new data at the General Session & Exhibition of the International Association for Dental Research (IADR), to be held in conjunction with the annual meetings of the American Association for Dental, Oral, and Craniofacial Research (AADOCR) and the Canadian Association for Dental Research (CADR). The biennial gathering of the three organizations will take place March 13-16 at the Ernest N. Morial Convention Center in New Orleans, Louisiana.

In two oral presentations and two poster discussions, researchers will present data supporting the clinical effectiveness of several of the Company’s iconic products, including Listerine®1 Cool Mint and Cool Mint Zero Alcohol mouthwashes, Listerine® Clinical Solutions Antiseptic Gum Health Mouthwash, and Listerine® Clinical Solutions Breath Defense Mouthwash. Plaque-Free Sites in a 12-Week Supervised Mouthrinse/Floss Trial (Milleman KR, et al), presented by lead author Kimberly Ruth Milleman, RDH, BSEd, MS, PhD, Global Clinical Director of Salus Research, Inc., in Indiana.

This randomized trial tested the plaque-controlling efficacy of a flossing and/or brushing regimen with an essential oil mouthwash. In evaluating the percentage of whole-mouth plaque-free sites at weeks 4 and 12, statistically significant differences favored groups whose regimens included mouthwash.

Flossing Time/Technique Competency in Supervised Plaque, Gingivitis Clinical Trial (Bosma ML, et al), presented by author and Kenvue R&D Manager Gabriella John. This 12-week, virtually supervised randomized trial found that training in American Dental Association flossing techniques, coupled with virtual dental hygienist supervision, improved participants’ flossing skills. In addition, it found that an essential oil mouth rinse helped compensate for remaining flossing insufficiencies, ultimately improving gingival health and reducing interproximal plaque.

Efficacy of Essential Oil and Zinc-Containing Mouthrinse on Plaque Microbiome, presented by lead author Kyungrok Min, PhD, Principal Scientist at Kenvue. This study used quantitative microbiome analysis to determine that mouth rinses containing a fixed combination of four essential oils (EO) and zinc were effective in reducing plaque and gingivitis and creating a healthier plaque microbiota.

Mouthrinse Effect on Oral Malodor Using Gas Chromatography/Organoleptic Judges (Milleman J, et al), also presented by lead author Kimberly Ruth Milleman, RDH, BSEd, MS, PhD, Global Clinical Director of Salus Research, Inc., in Indiana. This single-center randomized, placebo-controlled trial confirmed the ability of an experimental mouth rinse to neutralize oral malodor by reducing volatile sulfur compounds, the primary source of bad breath. The mouth rinse contained essential oil, zinc, and an odor-neutralizing technology.
