Seven Outreach Trips to Underserved Communities Around the World Mark a Decade of Henry Schein Cares Supporting NYU Program

More than 3,000 people in four different countries – 2,049 children and 1,076 adults – received free dental care during the 2018-2019 school year, thanks to the NYU Dentistry/Henry Schein Cares Global Student Outreach Program. This year marks the 10th anniversary of the collaboration between NYU College of Dentistry (NYU Dentistry) and Henry Schein Cares, the global corporate social responsibility program of Henry Schein, Inc.

NYU Dentistry’s outreach program provides free dental services to populations facing barriers to oral health, including a lack of local dental providers, economic hardship, and inadequate health insurance coverage. Traveling to communities in interdisciplinary teams of general dentists and dental specialists, NYU Dentistry students receive hands-on clinical instruction and are exposed to public health issues under the guidance of faculty, alumni, local partners, and community leaders.

“The NYU Dentistry/Henry Schein Cares Global Student Outreach Program is designed with two goals in mind: delivering and sustaining oral health improvements in host communities and creating exceptional service-learning experiences for our students. Through our outreach, we are fulfilling a critical health need, conducting meaningful public health research, and transforming the way future oral healthcare providers view their roles in society,” said Stuart Hirsch, DDS, vice dean for international initiatives and continuing dental education at NYU Dentistry.

Henry Schein donates the oral health supplies and materials for each outreach program. The teams use these products to provide routine and urgent dental services, including screenings, preventive and restorative treatment for children and adults at no cost to the patient.

Reaching Underserved Communities Near and Far

Barriers to oral health exist both in the United States and abroad. This year, NYU College of Dentistry’s outreach program took teams to three domestic locations (Clinton County and Dutchess County in New York, and Machias, Maine) and four international locations (Phnom Penh, Cambodia; Cuenca, Ecuador; Surkhet, Nepal; and Pokhara, Nepal).

Last fall was the first time NYU Dentistry hosted a week-long clinic in Clinton County in northeast New York, on the border of Canada. Health insurance company United Healthcare recognized that many of its members in the North Country had not seen a dentist in the past year, so partnered with NYU Dentistry to fund a week-long clinic in Plattsburgh. Patients received dental care with no out-of-pocket cost. A team from NYU Dentistry will return in fall 2019.

This also marked the first year that the NYU Dentistry/Henry Schein Cares Global Student Outreach Program provided care in Cuenca, Ecuador, and Pokhara, Nepal. NYU Dentistry previously conducted outreach programs in Ecuador and Nepal, but returned to new cities based on local needs and partnerships.

For the second year, the NYU Dentistry/Henry Schein Cares Global Student Outreach Program held a program in Surkhet, a remote village in western Nepal near the border with India. Surkhet is the home of the Kopila Valley School and Children's Home, founded in 2015 by CNN Hero of the Year Maggie Doyne.

Building Sustainable Models of Care

As part of its outreach efforts, the NYU Dentistry/Henry Schein Cares Global Student Outreach Program works to implement sustainable oral health prevention, including identifying and training local members of the community who can continue efforts on an ongoing basis. For instance, in Cambodia, NYU Dentistry partners with University of Puthisastra’s Faculty of Dentistry, which is already successfully implementing oral health education, screening, and preventive care such as sealants and silver diamine fluoride (SDF). During the November 2018 outreach, NYU Dentistry helped with urgent dental treatment needs such as restorations, extractions, and root canals.

In addition, NYU Dentistry has started to transition out of providing clinical care in Kathmandu, Nepal, empowering local partners to take ownership of the program through classroom toothbrushing and referring children to a local dental college as a permanent dental home.

“We have been coming to Kathmandu since 2013, and have measured a marked improvement in the children’s oral health,” said Hirsch. “It is time for the sustainability to be entirely in the community's control in the coming years.”

Transforming Future Dental Professionals

Each year, nearly 200 NYU dental, dental hygiene, and postgraduate students participate in outreach programs. These intensive, community-based clinical experiences give students hands-on training and one-on-one clinical instruction from supervising faculty.

But beyond learning the skills to maintain healthy smiles, students also describe having transformative experiences, rethinking their roles as future health care providers and gaining a deeper understanding of barriers to healthcare in different settings.

“I have had the opportunity of entering communities and learning about the daily challenges others face that I simply take for granted. Lack of nutritious foods, access to a dental clinic, fluoridated water, and ability to afford toothpaste are just some of the obstacles people endure, keeping them from better oral health and a full smile,” reflected Nicole Haydt, who as a dental student traveled to Cambodia and Nepal with the NYU Dentistry/Henry Schein Global Student Outreach Program.

“Serving globally has allowed me to visualize a greater purpose for how I want to live my life, and work with a team to provide care for those most in need," added Haydt, who graduated from NYU Dentistry in May.

“The cultures and communities in which we served are varied and complex, as are the reasons why those who need dental care the most aren't receiving it,” said Catherine Lee, who participated in outreach trips to Ecuador and Maine as a dental student and also graduated in May from NYU Dentistry. “My dental global public health training at NYU has profoundly impacted how I think about my career in dentistry and has strengthened my sense of responsibility not only locally, but also to the global community.”

For the 2019-2020 school year, NYU Dentistry will send teams to Clinton County, NY; Pokhara and Surkhet, Nepal; Cuenca, Ecuador; and Machias, Maine. Other sites under consideration include Hudson, Windham, and Malone, NY; and Cape Town, South Africa.

Light therapy could replace opioids as main treatment for cancer treatment side effect

Worldwide research coalition offers new guidelines for healing oral mucositis in head, neck cancer patients

BUFFALO, N.Y. — A worldwide coalition of researchers and clinicians has agreed that light therapy is among the most effective interventions for the prevention of oral mucositis, painful ulcers in the mouth resulting from cancer therapy.

The new guidelines from the Multinational Association of Supportive Care in Cancer (MASCC) and International Society of Oral Oncology (ISOO), published on this month in the journal Supportive Care in Cancer, present a significant upgrade in care guidelines for adult cancer patients worldwide.

The guidelines recommend photobiomodulation therapy, a form of low-dose light therapy, for the prevention of oral mucositis caused by radiation therapy for head and neck cancer or stem cell transplantation.

“Many cancer patients can now benefit from this treatment, said Praveen Arany, DDS, PhD, co-corresponding author on the paper and assistant professor of oral biology at the University at Buffalo School of Dental Medicine.

“The staggering breadth of clinical application for photobiomodulation therapy has been both a boon and a bane for the field. Several anecdotal clinical reports have been plagued with inconsistent outcomes and questionable rationales, often relegating this treatment to a pseudoscience.

“However, recent advancements are enabling rigorous validation of clinical protocols. This is a major milestone for the field and we are confident it will set a clear path for several exciting clinical applications for photobiomodulation therapy from concussions and wound healing to exciting new work with regenerative medicine and stem cells,” said Arany, also president of the World Association for photobiomoduLation Therapy (WALT).

Multiple studies have found that patients report oral mucositis as the worst side effect of their cancer treatment. Pain from the condition can slow or delay cancer treatment, and in severe cases require hospitalization.

Light therapies have existed for decades, but improvements in the technology have made the treatment more affordable for wider use. At a high power, light, often in the form of a laser, is used in medicine to cut or destroy tissue. But at a low power, it has the ability to relieve pain or inflammation and promote healing.

The treatment is rising in use across Europe, Brazil, India, Canada and several other nations. The findings provide an upgrade to previous guidelines published in 2013, which noted the effectiveness of light therapy and recommended, based on relatively limited evidence at that time, the intervention as an optional therapy in specific cancer patient populations and settings.

The review was led by Zadik Yehuda, DMD, senior lecturer at the Hebrew University-Hadassah School of Dental Medicine in Jerusalem, Israel; and Sharon Elad, DMD, chair of the MASCC/ISOO Mucositis Study Group. Along with 14 other global experts, they triaged hundreds of research papers published on photobiomodulation therapy for oral mucositis.

“These updated guidelines will provide health care professionals with better tools to deliver care for cancer patients, said Elad, also professor at Eastman Institute for Oral Health at the University of Rochester Medical Center.

“But even with the best evidence-based interventions, we don’t yet have an ultimate guideline for mucositis in all clinical settings. We look forward to future research to help shape a more universal implementation of photobiomodulation therapy as well as identify additional effective and validated protocols.”

Among other findings, the investigators identified five new protocols, recommending light therapy for the prevention of oral mucositis in stem cell transplant patients, and head and neck cancer patients receiving radiation therapy with or without chemotherapy. No major short-term side effects of light therapy were reported.

The therapy could potentially serve as an alternative to opioids, often prescribed to alleviate the symptoms of oral mucositis, says Arany.

Light therapy was also the subject of a recent congressional hearing before the House Science, Space and Technology Committee in Washington, D.C. The briefing, held in October 2018, invited a panel of international experts to discuss the potential of photobiomodulation to improve health care and lower dependence on opioids.

Future studies are needed to verify the effectiveness of light therapy in managing oral mucositis in pediatric cancer patients and in adult cancer patients receiving only chemotherapy.

60 Years of Engaging Next Generation Dental Leaders: The AADR and Dentsply Sirona Present the 2019 SCADA Awards

The 2019 winners of the SCADA program were recognized for their research achievements during the IADR/AADR/CADR General Session & Exhibition in Vancouver, Canada, on June 19th, 2019. This time, 69 student clinicians representing 28 countries participated in the SCADA program. In 1959, Dentsply Sirona and the American Dental Association initiated the competition for dental students in the United States. Since 2017, AADR has co-sponsored the research competition with Dentsply Sirona. The SCADA program has two main goals: Firstly, to support the next generation dental professionals in discovering the importance of dental research, and secondly to offer expanded networking opportunities with clinicians and scientists from all over the world to advance their skills and careers. The overall goal is to advance science and improve global oral health. Moreover, SCADA Alumni and the participants of national and global competitions become part of a global community of dental professionals. 

60 years SCADA awards – looking back with pride 

During the last 60 years, SCADA has grown to become a globally recognized program with more than 8,400 participants from 28 countries. “Dentsply Sirona is proud to support the next generation of leaders in dentistry early in their careers. And we sincerely hope this is the beginning of a long and meaningful collaboration with these dental leaders throughout their professional lives. Personally, I am so impressed by the students’ professionalism, intellectual curiosity and high level of research. Congratulations to all of the participants in this year’s SCADA program! SCADA Competition – a win-win for participants and judges 

The ceremony in Vancouver recognized the six winners of the SCADA US competition. The 57 US students previously competed within their respective dental schools and were selected to participate in the SCADA competition. They presented their research to an international panel of experts in one of two categories: Clinical Research and Public Health or Basic and Translational Science Research.

Deepti Karhade, Harvard School of Dental Medicine and winner of the 2nd Prize the Category Clinical Research and Public Health, sums up her impressions: “Being part of SCADA is not only that you are a part of the competition but also that you collaborate with peers from all around the world. I think my favorite part while being here is that I am realizing that I am not the only one being passionate about the things I am working on.” Dr. Chuck Schuler, Professor and former Dean at the University of British Columbia and one of the judges, adds: “SCADA is really important to demonstrate why research is so important for dental education, because the students have to appreciate that things are changing. Thus, they have to keep looking for the newest advances and have to incorporate them in their practices for the most improved care for their patients.” 

Beyond the SCADA Competition – four additional awards 

In addition, the winners of 12 programs throughout the world had the opportunity to join the IADR/AADR/CADR General Session to present their research on June 20th, 2019. Moreover, SCADA provides fellowship funding to promote continued scholarship and help support graduate education. Two post-doctoral students were awarded Henry M. Thornton SCADA Fellowships; Dr. Adam Lietzan from the University of North Carolina at Chapel Hill and Dr. Nadeem Bjile from the University of Hong Kong. And the ADA Foundation/Dentsply Sirona Research Award for Dual Degree Candidates was awarded to Dr. Drake Williams in support of his doctoral research at the University of California, Los Angeles.

The SCADA Board of Directors also supports and acknowledges leaders for their commitment to the advancement of dentistry through two additional awards. Dr. Russell Giordano received the Burton C. Borgelt / SCADA Faculty Advisor Award for his commitment to mentoring students at the Boston University Henry M. Goldman School of Dental Medicine. Dean Sharon Gordon from the University of Connecticut was honored with the Alan J. Davis / SCADA Achievement Award for outstanding service to SCADA and the dental profession.

Journal of Dental Research Centennial July 2019: Fluoride Revolution and Dental Caries

2019 marks the Centennial of the Journal of Dental Research (JDR). Over the last century the JDR has been dedicated to the dissemination of new knowledge and information on all sciences relevant to dentistry and to the oral cavity and associated structures in health and disease. To celebrate, the JDR is featuring a yearlong, monthly commemorative article and podcast series that highlights topics that have transformed dental, oral and craniofacial research over the past 100 years.

While the global epidemic of dental caries that began about 140 years ago was very largely caused by the rise in sugar consumption, the more recent decline in caries during the last 50 years has been due largely to the use of fluoride. In the second July 2019 issue of the JDR, the article "Fluoride Revolution and Dental Caries: Evolution of Policies for Global Use," by IADR past president Helen Whelton, University College Cork, Ireland, John Spencer, University of Adelaide, Australia, Loc Do, University of Adelaide, Australia and Andrew Rugg-Gunn, Newcastle University, Newcastle upon Tyne, England, focuses on population-level interventions, which have been predominantly through fluoridation of water supplies and the widespread use of fluoride toothpaste.

"Epidemiological studies over 70 years ago provided the basis for the use of fluorides in caries prevention and revealed the clear relation between fluoride exposure in drinking water and the prevalence and severity of dental fluorosis and dental caries," said Spencer." The proposition that cities with water supplies deficient in fluoride might have their level brought up to inhibit caries emerged in 1943 and 1944. This hypothesis was tested in four community fluoridation trials in the United States and Canada. Those findings showed a marked reduction in caries experience, around 50%, in children and adolescents in the fluoridated cities compared to non-fluoridated control cities or the levels of caries in cities before fluoridation."

"Previously drinking water had been the only significant source of fluoride. Now there are additional sources, most notably fluoridated toothpaste. These two fluoride sources have an additive effect. In many countries in the early 1960s, 12-year-olds had on average 5 decayed permanent teeth, by age 15 it had gone up to 9, this figure is now less than one for 12-year-olds," said Whelton. "Policy has remained supportive of the use of both community fluoridation where practicable and supportive of affordable fluoridated toothpaste. Fluoride policies have radically improved oral health, enhancing general health and quality of life for populations across the world."

The seventh JDR Centennial podcast, titled "Fluoride Revolution and Dental Caries: Evolution of Policies for Global Use" features a conversation between Spencer, Whelton and Joy Richman, University of British Columbia, Vancouver, Canada.

Throughout 2019 JDR Associate Editor, Nicholas Jakubovics, Newcastle University, England, shares 'Historical Highlights' and archival excerpts from the rich history of research findings published in the JDR. In the second July issue, Jakubovics highlights the article "Bovine Teeth as Possible Substitutes in the Adhesion Test" (Nakamichi I., Iwaku M., Fusayama T. 1983. Bovine Teeth as Possible Substitutes in the Adhesion Test. J Dent Res. 62:1076-1081). Through the 1950s and 1960s, the consumption of sugar rose to peak levels around the world, while a shortage of dentists left many people without adequate dental care. Intact human enamel was becoming more difficult to obtain for scientific research studies and alternatives needed to be found. This influential paper directly compared human and bovine enamel as substrates for adhesion and found essentially no significant differences between the two. This work paved the way for the widespread use of bovine enamel as a substitute for human enamel and enabled research that otherwise would not have been possible.

The Kois Center for Dental Research and Education Launches New App for Patients: Healthy Mouth

The Kois Center recently launched a new self-assessment, educational app specifically designed for patients called the Healthy Mouth. The app provides users with an immediate estimate of their oral health using an algorithm that analyzes user responses to oral health questions, before providing risk estimates in a variety of categories. Access to the Healthy Mouth app is available for the iPhone in the App Store.

"We are thrilled to launch a product that provides a powerful educational tool for patients everywhere," said Kois Center CEO, Mr. Johnny Kois. "Proper dental care starts with proper dental awareness."

The Healthy Mouth app features a series of images with YES and NO questions and answers to help patients evaluate their oral health. The app screens for issues with gum health, overall teeth health, bite and jaw-joint health as well as smile characteristics. After the oral health self-assessment, the app helps users locate qualified dentists near them that are equipped to more deeply interpret the app results. The entire app-screening process is designed to be quick, from evaluation to appointment scheduling, and is often completed in less than seven minutes.

The Healthy Mouth app provides users access to listed dentists who practice using a specific set of techniques and protocols that are based on the latest science. These protocols are proven to be effective for maintaining and achieving optimal oral health and are taught to practicing dentists at The Kois Center. All Kois-trained dentists are qualified to assess the patient results received from the Healthy Mouth app.

Benefitting The Cure Starts Now, a non-profit organization committed to the fight against cancer, the dentists and members of their staff met with attendees where they spoke about their commitment to providing comprehensive, patient-centric oral health care. The event, which included a 4 mile run/walk, cycling races, and a beer, wine and food festival, brought in over 15,000 Cincinnati residents.
