Okayama University research: Possible link between excess chewing muscle activity and dental disease

Posted on Monday, September 10, 2018

(Okayama, 10 September) Researchers at Okayama University report in Archives of Oral Biology that involuntary masseter muscle activity — unnecessary motion of the muscles normally used for chewing — may be linked to a common dental disease known as periodontitis.

Periodontitis is an infection of the gum (the flesh around the roots of teeth in the upper and lower jaws), damaging jaw tissue and ultimately destroying the bone holding teeth in place, resulting in tooth loosening and loss.  Medical researchers have been investigating the relationship between periodontitis and the activity of the masseter muscles — the main muscles involved in the process of chewing. Specifically, it has been unclear whether involuntary teeth grinding and jaw clenching, a condition known as bruxism, is related to periodontitis and its development. Now, Professor Shogo Minagi and Dr. Seiya Kato from Okayama University and colleagues have performed detailed measurements of masseter muscle activity in a group of people with various degrees of periodontal disease, and found that non-functional teeth grinding might be related to the acuteness of periodontitis.

Bruxism not only occurs when a person is awake, but also when sleeping.  Therefore, the researchers made sure to design the study so that it provides information on masseter muscle activity over a long-enough period (daytime followed by night-time).  A group of 31 people was selected; 16 of them had no or mild periodontitis (the ‘NMP’ group), while 15 had moderate to severe periodontitis (the ‘MSP’ group).

Each individual was equipped with an ambulatory portable device measuring muscle activity via a technique known as surface electromyography.  (The technique involves placing electrodes on the skin lying over the muscle; muscular motion triggers an electrical signal which is recorded.)  Participants of the study had to maintain a diary noting their activities including eating meals, which helped the researchers analyzing the data and filtering out muscular activity not coming from involuntary teeth grinding.  Teeth movement due to speech was filtered out by monitoring voice activity by a microphone attached to the electrodes.

Professor Minagi and colleagues found that there are differences in masseter muscle activity between the NMP and MSP groups: the accumulated duration of non-functional (i.e. not related to eating or talking) high-intensity muscle motion was significantly longer for the participants from the MSP group.  The finding applies to both awake and asleep regimes.  The scientists concluded that “masseter muscle activity might be related to the severity of periodontitis”, but they stress that causation — bruxism leading to periodontitis — cannot be concluded from the study.  They also point to limitations of their study: oral conditions like missing teeth or the use of removable partial dentures not being taken into account, and the limited capabilities of the ambulant surface electromyography setup.

Background

Bruxism.  Periodontitis

Bruxism is the term for non-functional, habitual teeth grinding or jaw clenching.  (Eating and talking are functional activities.)  Two types of bruxism are distinguished: when a person is asleep (nocturnal bruxism) and when awake.  Symptoms typically associated with both types of bruxism include hypersensitive teeth, headaches, damage to dental fillings and tooth wear.

It is an open question whether there is a relationship between bruxism and periodontal disease — the gum infection damaging soft tissue surrounding teeth and destroying the bones supporting them.  Periodontitis is a common disease, but can be prevented, mainly by observing good oral hygiene — daily brushing and flossing teeth combined with regular dental checkups.

Professor Minagi and Dr. Kato from Okayama University and colleagues have now investigated the potential relationship between bruxism and periodontitis, by monitoring teeth grinding activity of people at different stages of periodontal disease.  They found that muscular teeth-grinding motion might be related to the severity of the disease.

Electromyography

Electromyography (EMG) is a medical technique used for recording the electrical activity resulting from skeletal muscle activity. EMG involves an instrument called an electromyograph, which produces data in the form of a so-called electromyograph.  In surface EMG, the electrodes that pick up the electrical activity are placed onto the skin, as opposed to intramuscular EMG, where electrode needles are inserted directly into the muscle, under the skin.

Professor Minagi and colleagues used surface EMG — which does not hurt as there is no need to pierce the skin — to detect activity of the masseter muscle in a group of people suffering from periodontitis at various levels of severity.  The design of their medical study enabled monitoring muscle activity for long periods of time (roughly a daily cycle) and helped to provide insights into the relationship between involuntary teeth grinding (bruxism) and the degree of periodontitis.

Carestream Dental’s Latest Updates to CS 3600 Acquisition Software Smooths the Scanning Process

Posted on Friday, September 7, 2018

Carestream Dental is previewing its latest update to the CS 3600 intraoral scanner acquisition software: Version 3.1.0* at CDA North. Known for its high-speed, yet accurate, digital impression scanning, the CS 3600 includes several new customer-driven features aimed at improving the user experience while improving efficiency.

Hybrid Scan: The new Hybrid scan feature is a patent-pending evolutionary workflow that merges the scanned-in vivo data with the scanned data from a partial conventional impression into a single file. This significantly improves the quality of the capture of subgingival margin data for a more ideal design and fit of the final prosthetic.

“We often hear from practitioners that detecting margin lines when blood or deep subgingival margins are present is a challenge when capturing a digital intraoral scan,” said Edward Shellard, D.M.D., chief dental officer of Carestream Dental. “To alleviate this problem, and improve the scanning process for CS 3600 scanners, we developed the Hybrid scan.”

Improved Acquisition Speed: Time is of the essence when capturing a scan. With version 3.1.0, the total amount of time it takes to acquire and process a digital impression is up to 22% faster, depending on the acquisition computer used.**

Ability to Add Sound: With version 3.1.0, users now also have the option of selecting an audible notification when the scanner is actively scanning and acquiring data. The scanning sound can be set to a scanner beep or a PC sound via an uploaded WAV file of the doctor’s choice.

“Practitioners can create a relaxing environment with soothing songs or add excitement to their scanning routine with upbeat music—the possibilities are endless,” said Shellard.

Preparation Check: To make the restorative workflow even more efficient, the acquisition software now features Preparation Check, which allows for superimposition of the pre- and post-preparation scans to perform distance measurements.

Streamlined Sleep Medicine Workflow: Doctors practicing dental sleep medicine will benefit from the Multiple Bite Capture option, which allows for the capture of multiple bite registrations necessary for designing and manufacturing sleep apnea devices. Up to three different bite registrations can be captured and viewed in the software: for example, normal, open and protruded bites. The bite matrix files can be exported and sent with the dataset to the lab or device manufacturer and viewed in the CAD software.

“This unique new patent-pending workflow is included in this update as a response to feedback we received from current CS 3600 scanner customers and who are practicing sleep dentistry and sleep device manufacturers,” said Shellard. “It simplifies the workflow and streamlines the process, allowing the practitioner to capture multiple bites in a single capture. An additional benefit is that the lab receives a single file with all the information needed to create the sleep device.”

To facilitate the fabrication of sleep devices even further, the CS 3600 is certified for use with Panthera Sleep, ProSomnus Sleep Technolgoes and SomnoMed.

Efficient Workflow Selection: The Select Acquisition Type window has been redesigned to better segment the four distinct workflows, including Orthodontics, Implant, Restoration, and Import. Users can see an expanded menu of options specific to each workflow by hovering over their workflow of choice. The Measurement tool is also now available in all three workflows—restorative, orthodontic and implant—allowing practitioners to take measurements from any workflow for any indications.

Updated Preferences: Version 3.1.0’s Preferences options have been redesigned to have the same appearance and functionality as the Preferences in the new Carestream Dental Imaging software version 8. This provides more continuity across Carestream Dental’s imaging software for a more consistent experience.

Software updates—such as this one—are included for customers enrolled in a CS Advantage plan. For customers who are not currently enrolled in a CS Advantage plan, one-time updates are available.

Also being previewed at CDA North is Carestream Dental’s new milling system, the CS 3100.* The system is the latest addition to the CS Solutions portfolio and, when combined with the CS 3600, offers doctors complete HD chairside milling.

To learn more about the latest updates to the CS 3600 or any product in the CS Solutions portfolio, visit Carestream Dental in booth #1112. To learn more about Carestream Dental’s innovative technology call 800.944.6365 or visit carestreamdental.com.

* Work in Progress, not available for sale

**Internal test conducted using automated testing toll which simulated the 3D mesh construction and display process. Scan speed improvement is between 13-22% and varies by system configuration. Speed improvement does not factor in scanning skills.

Carestream Dental’s New CBCT System Drives Precision Through Intelligence

Posted on Thursday, September 6, 2018

Today at CDA North, Carestream Dental launched an all-new imaging system: The CS 9600 cone beam computed tomography (CBCT) system, which enables new levels of image quality, usability and, ultimately, precise diagnostic capabilities.

Featuring intelligent automation, first-time-right imaging technology and highly simplified workflows, this system ensures reliable patient positioning, reproducible results and exceptional images. For a future beyond today, the CS 9600 can also grow with a doctor’s practice—thanks to upgradeable fields of view, versatile imaging options and intuitive software.

Blending 2D panoramic technology, CBCT scanning, 3D model/impression scanning1 with 3D facial scanning,2 the CS 9600 can also be upgraded to include cephalometric imaging.2 Three different editions, featuring 10, 12 and 14 fields of view, can be upgraded to a larger FOV without replacing the unit—ensuring that the system covers all the needs of practices that place implants, perform oral and maxillofacial surgery or perform orthodontic, periodontal or endodontic procedures.

“Digital dentistry is in a constant state of change,” Ed Shellard, D.M.D, chief dental officer, Carestream Dental, said. “We not only want to enable doctors to keep up with these changes, but we also want them to use these innovations to their fullest potential. The CS 9600 is an intelligent and powerful imaging system that allows users of any experience level to capture high-quality diagnostic images.”

A prime example of the CS 9600’s intelligent features is the SmartPad, an intuitive touchscreen interface that guides the operator through the exam and simplifies procedures from exam setup to image quality control. The unique live positioning assistant utilizes cameras to properly position the patient—with no need for laser beams. Combined with optional Scout scan and face-to-face positioning, this unique technology helps the operator capture the right image the first time. The operator can also verify the image quality immediately after the acquisition on the high-resolution touch screen using quality control tools.

The CS 9600’s SmartAuto technology further enhances workflow automation and exam precision. SmartAuto Pan3 captures the patient’s jaw shape and position in three dimensions and automatically calculates the correct exposure settings and trajectory, while SmartAuto 3D3 enables users to precisely define the field of view position on a low dose scout view, virtually eliminating the risk of incorrect positioning for repeatable image quality each time.

Images are further enhanced thanks to the CS 9600’s patented CS MAR3 technology. Carestream Dental’s high-end MAR algorithm provides specific metal artifact reduction and a unique tool to dynamically compare images with and without the MAR filter; helping to confirm diagnosis and reducing the risk of misinterpretation.

The CS 9600 also helps to address concerns regarding patient safety and adherence to the ALARA (as low as reasonably achievable) principle. With its low dose programs, the CS 9600 delivers 3D images at a significantly lower dose than 2D panoramic imaging; helping to protect patients.

Another new feature to the CS 9600 is an integrated retractable seat,3 which enables users to scan patients either in a comfortable seated position when higher stability is required or rotated out of the way when not in use or to accommodate patients in wheelchairs.

“These unique features all add up to deliver what every doctor is seeking: Exceptional image quality,” Shellard said. “When you can position the patient better the first time; capture images faster; and not have to worry about things like metal artifacts, it ultimately means you’re getting the best image, every time.”

The CS 9600 is supported by Carestream Dental’s newest imaging platform: CS Imaging version 8. This updated software enables one-stop access to all 2D images, 3D images and CAD/CAM data through a single system. Additional modules, such as the Prosthetic-Driven Implant Planning module, CS Airway, CS Model and CS Model+, can be integrated with CS 9600 to expand diagnoses and treatment planning capabilities. The CS 9600 will also be fully integrated into Carestream Dental’s suite of practice management software including CS PracticeWorks, CS SoftDent, CS OrthoTrac and CS WinOMS.

Carestream Dental’s CS Advantage comprehensive service offering further enhances doctors’ CS 9600 investment. For example, the innovative CS UpStream advanced monitoring service continuously screens the CS 9600 system and—by monitoring its historical behavior—prevents downtime and maximizes system availability. CS Advantage members also get extended warranty coverage, software updates, training and support to ensure the best ownership experience.

New Hu-Friedy Waterline Filter Helps Dental Practices Continuously Deliver Clean Water With Minimal Effort

Posted on Thursday, September 6, 2018

Hu-Friedy, the global leader in dental instrument manufacturing and infection prevention solutions, today announced the release of the HU-FRIEDY WATERLINE FILTER, the company’s newest innovation that will help dental practices continuously deliver clean water to patients.

“Dental unit waterlines are known as notorious breeding grounds for bacteria, which can form into huge, persistent colonies of sticky biofilm. But traditionally, waterline filters can be tricky to install, burdensome to maintain, and inconvenient to change,” said Jennifer Naylor, Executive Vice President at Hu-Friedy. “We set out to create a product that will help keep waterlines clean while requiring minimal effort from staff.”

The HU-FRIEDY WATERLINE FILTER is registered by the EPA in all 50 states as a microbial agent for use in dental unit waterlines attached to dynamic dental instruments. It successfully reduces contamination and odor-causing bacteria to 200 CFU/mL or less — a standard that far exceeds the CDC recommended level of 500 CFU/mL. Moreover, the filter provides ongoing, consistent, and accurate dosing to inhibit microbial contamination for up to a year.

In addition to longevity, the waterline filter offers maximum convenience. The ready-to-use cartridge can be installed in five minutes and requires no daily, weekly, or monthly maintenance. Its universal design is the first of its kind to be suitable for both municipal waterlines and independent water reservoirs. Because the filter is non-corrosive and does not contain silver or other hazardous materials, disposal is quick and easy.

The HU-FRIEDY WATERLINE FILTER is the latest addition to Hu-Friedy’s family of waterline solutions, which includes VistaTab™ for periodic cleaning, the VistaClean™ daily irrigant for protecting waterlines from deposits and scale, and Team Vista™ which provides the complete system.

Carestream Dental Marks First Year as Freestanding Digital Dental Technology Company

Posted on Wednesday, September 5, 2018

Carestream Dental is breaking with tradition for a good reason. The traditional one-year anniversary gift may be paper, but the independent dental technology company can’t help but celebrate with all things digital.

“It’s been an exciting first year for Carestream Dental, full of transformation and growth,” Lisa Ashby, CEO, Carestream Dental, said. “I’m proud of all the company and its employees have achieved, and I’m grateful for the doctors and partners around the world who trust us to deliver the solutions they need to change their patients’ lives.”

Carestream Dental became a freestanding dental technology company focused solely on digital solutions on Sept. 1, 2017, when it was acquired by Clayton, Dubilier & Rice (CD&R) and CareCapital Advisors Limited. The transition has made the company more nimble and better able to respond to shifting corporate dynamics and new patient demands in the field of oral healthcare.

“We are very proud of our close collaboration with the Carestream Dental team, which over the past year has strengthened our belief that the company is extremely well positioned to meet the dynamic needs of the global oral healthcare industry with innovative products and high-quality services,” Derek Strum, CD&R Partner, said.

In its first year as a freestanding company, Carestream Dental delivered several all-new solutions thanks to continued investment in R&D and commitment to the voice of the customer. This technology helps doctors make more confident diagnoses, perform well-planned procedures, provide better patient communication and offer faster treatments.

-New intelligent software updates to the CS 3600 intraoral scanner

-A native cloud practice management software suite that keeps dental practice from getting in the way of dental care

-Significant updates to CS OrthoTrac v14, including a universal clear aligner tracker

-A software module that redefines the orthodontic workflow, CS Model+

As a testament to the quality and innovation of Carestream Dental’s solutions, its existing technology also received industry recognition this year. The CS 8100SC 3D imaging system was honored with an Edison Award and the CS 3600 intraoral scanner outperformed competitors, as well as traditional impressions, in a 2018 study on finish line accuracy. The company also surpassed a significant sales milestone by shipping out the 20,000th unit of the CS 8100 Family.

In yet another twist on tradition, Carestream Dental isn’t expecting any presents for its one-year anniversary. Instead, the company is gifting the oral healthcare community with a series of thought leadership ebooks throughout the month of September in honor of its anniversary. They can be downloaded at carestreamdental.com/ebooks.

“We don’t want to just celebrate the company’s successes over the past year but celebrate the doctors, team members and industry partners who helped make this year such a success” Ashby said. “These ebooks turn the idea of the ‘paper’ one-year anniversary gift on its head by taking a digital approach.”

This may be Carestream Dental’s first year as an independent digital dental technology company, but its roots go deep. Eastman Kodak, Trophy and PracticeWorks are all legacy brands that make up the company’s 120 year-history. Looking forward, Carestream Dental will continue to transform dentistry, simplify technology and change lives for another century or more.
