White Paper Stresses Importance of Oral Health Care Benefit for Older Adults

Oral Health America (OHA), along with a diverse group of partners, released a white paper, An Oral Health Benefit in Medicare Part B: It’s Time to Include Oral Health in Health Care. This white paper is an interprofessional, collaborative effort written and published by leaders in the consumer, healthcare and dental fields, including Oral Health America, the American Dental Association, Center for Medicare Advocacy, the DentaQuest Foundation, Families USA, Justice in Aging and the Santa Fe Group.

With an expected 72.1 million seniors living in the United States by 2030, An Oral Health Benefit in Medicare Part B: It’s Time to Include Oral Health in Health Care, examines the need for oral health care to be integrated with, and elevated to, the same importance as the rest of health care in Medicare.

"The lack of a dental benefit in Medicare affects older adults overall health, quality of life and financial stability," said Beth Truett, President & CEO of OHA. "To ensure access to care, we must provide coverage to pay for care. OHA is focused on finding equitable solutions to increase the overall health and wellness of America’s seniors. We believe that including a publicly funded benefit in Medicare Part B is a solution that values oral health as key to total health."

Medicare plays a key role in providing health and financial security to 59 million older people and younger people with disabilities. However, traditional Medicare does not include coverage for routine oral health care like checkups, cleanings and x-rays, or restorative procedures like fillings or bridges, tooth extractions and dentures.

“Medicare's dental exclusion is misguided given the clear connection between oral health and overall health,” said Wey Wey Kwok, JD, Senior Attorney at the Center for Medicare Advocacy. “The time has come to include an oral health benefit that covers preventive services, disease management and necessary procedures for all Medicare beneficiaries.”

An Oral Health Benefit in Medicare Part B: It’s Time to Include Oral Health in Health Care looks at many aspects of adding an oral health benefit to Medicare, including medical necessity, costs and the need for legislative changes. Top findings include:

-70 percent of all Medicare recipients lack or have limited dental insurance and fewer than half access dental care each year.

-Cost is the number one reason that older adults have not gone to the dentist in the past year.

-Integrating dental coverage in Medicare would close disparities in dental use and expense between the uninsured and insured and among older adults with few financial resources and limited oral health education.

-Surveys show that consumers widely support adding oral health coverage to Medicare and prioritize two categories of care: checkups and pain treatment.

-71.2 percent of dentists agree that Medicare should include comprehensive dental benefits and a majority indicated they were willing to comply with typical Medicare practice requirements.

-The ADA recently commissioned a study that analyzed the cost structure for various dental benefit designs within Medicare. This study estimated that a comprehensive benefit without dollar value caps would cost the federal government between 31.4 billion dollars in 2016 dollars, $32.3 billion in 2018; the estimated base premium increase for a Part B benefit would be $14.50 per beneficiary per month. ADA input to this white paper does not constitute endorsement of inclusion of a dental benefit under Medicare at this time. The ADA is currently investigating a number of options to serve the dental care needs of a growing elder population.

An Oral Health Benefit in Medicare Part B: It’s Time to Include Oral Health in Health Care recommends the addition of a comprehensive oral health benefit to Medicare Part B as it covers outpatient services. Such a benefit would be amended to include dental services using the medically necessary and reasonable standard that applies to all Part B services. Advantages to the inclusion of Part B include:

-Ensures that everyone enrolled in Medicare will receive the oral health benefit.

-Provides the greatest number of beneficiaries access to a basic level of oral health care, encouraging equitable health solutions and provider participation.

-Simplifies a potentially confusing program and process for providers and beneficiaries.

-Uses established protections for both Medicare beneficiaries and providers, alleviating the need for a new system and bureaucracies.

To move an oral health benefit in Medicare Part B forward, Congress must pass legislation to remove the statutory exclusion in Section 1862(a)(12) of the Social Security Act. They also must establish dental coverage in Part B, permit payment for preventive services prescribed in the dental benefit and define the dental services in the Medicare Statute.

“Millions of older Americans lack the resources to maintain their oral health over their lifetime, which will negatively impact their general health and well-being,” said Elisa Chavez, DDS, Scholar of the Santa Fe Group. “[We are] one of many groups advocating for federal legislation to remove the dental exclusion in Medicare in order to make possible a dental benefit and a more coordinated, cost effective model of comprehensive health care.”

American Academy of Periodontology to Hold its 104th Annual Meeting in Vancouver

The American Academy of Periodontology (AAP) will hold its 104th Annual Meeting Oct. 27-30, 2018, at the Vancouver Convention Centre in Vancouver, British Columbia. The event is presented in collaboration with the Canadian Academy of Periodontology, the Japanese Academy of Clinical Periodontology, and the Japanese Society of Periodontology. All dental professionals—including students, early-career periodontists, hygienists, office staff, and members of the dental media—are encouraged to attend.

This year’s meeting, which provides up to 25.5 continuing education credits, features 45 courses and more than 30 new speakers. Ten redesigned program tracks include oral pathology, oral medicine, and oral diagnosis; emerging concepts and innovative therapies; implant surgery and prosthetic rehabilitation; and periodontal plastic and soft tissue surgery.

“With a roster of periodontics’ sharpest minds leading our many courses and events, this year’s Annual Meeting is set to be a gathering of our specialty’s best and brightest from around the world,” says Steven R. Proceedings from the 2017 World Workshop on the Classification of Periodontal and Peri-Implant Diseases and Conditions: Members of the Workshop’s organizing committee will discuss findings from the recently updated disease classification. This ticketed event, which takes place on Oct. 29, will highlight the newly developed staging and grading model for classification as well as the update’s implications on patient care and dental education.

All-new Speaker Studio: Located in Exhibit Hall booth 533, the Speaker Studio provides a unique opportunity for attendees to interact with speakers and moderators throughout the meeting. Course presenters will be on hand for post-session conversations and Q&A in an intimate, small-group setting.

Penn Dental Medicine Study Finds Dental Plaque No Match for Catalytic Nanoparticles

Combine a diet high in sugar with poor oral hygiene habits and dental cavities, or caries, will likely result. The sugar triggers the formation of an acidic biofilm, known as plaque, on the teeth, eroding the surface. Early childhood caries is a severe form of tooth decay that affects one in four children in the U.S. and millions globally. It's a particularly severe problem in underprivileged populations.

In a study published in Nature Communications, researchers led by Dr. Michel Koo, Professor at Penn Dental Medicine, showed that twice-daily rinses with FDA-approved nanoparticles and hydrogen peroxide effectively disrupted biofilms and prevented tooth decay in both an experimental human-plaque-like biofilm and in an animal model that mimics early-childhood caries.

The nanoparticles break apart dental plaque through a unique pH-activated antibiofilm mechanism.

"The nanoparticles act as a peroxidase, activating hydrogen peroxide, a commonly used antiseptic, to generate free radicals that dismantle and kill biofilms in pathological acidic conditions but not at physiological pH, thus providing a targeted effect," says Koo.

Because the caries-causing plaque is highly acidic, the new therapy can precisely target pathogenic biofilms without harming the surrounding oral tissues or microbiota.

The particular iron-containing nanoparticle used in the experiments, ferumoxytol, is already FDA-approved to treat iron-deficiency, a promising indication that a topical application of the same nanoparticle would also be safe for human use.

"We used plaque samples from caries-active subjects to reconstruct these highly pathogenic biofilms on real human tooth enamel," says Koo. "This simulation showed that our treatment not only disrupts the biofilm but also prevents mineral destruction of the tooth's surface."

As further evidence of the treatment's targeted effect, the researchers found no significant change in the microbiome in the mouth after therapy and no signs of tissue damage.

Incorporating nanoparticles in a rinse or toothpaste could be a cost-effective way to significantly improve their effectiveness, says Koo. With evidence backing this approach in both an animal model and a human-like model of tooth decay, the research team is working to test its clinical efficacy.

Koo was recently awarded the IADR William H. Bowen Research in Dental Caries Award for his consistent record of developing innovative ways to treat caries.

AAOMS to honor its 100th anniversary at its Annual Meeting in Chicago

The American Association of Oral and Maxillofacial Surgeons (AAOMS) will celebrate its centennial anniversary during its 100th Annual Meeting, Scientific Sessions and Exhibition being held Oct. 8 to 13 in Chicago, Ill.

More than 4,000 oral and maxillofacial surgeons, faculty, residents and others are expected to gather in the city where the association was originally founded. In 1918, 29 doctors convened to hold the first meeting of the American Society of Exodontists, the organization that later became AAOMS.

The AAOMS Annual Meeting – the largest and most comprehensive educational conference in the world exclusively for oral and maxillofacial surgeons and their staff – will feature approximately 170 educational programs at the Hilton Chicago and McCormick Place West. Sessions will expound on such timely topics as the treatment of war injuries, advancements in cancer care and emerging technologies in maxillofacial nerve injuries.

Among other highlights of the centennial celebration are:

-A history museum will showcase artifacts related to the specialty – such as surgical mallets and cheek retractors – a historical timeline of association milestones and videos of interviews of past association presidents.

-A “100 Years of AAOMS” course will summarize the specialty’s history and discuss the evolution from exodontists to oral and maxillofacial surgeons.

-Former Secretary of State and Chairman of the Joint Chiefs of Staff, General Colin L. Powell, USA (Ret.), will deliver the meeting’s keynote address, “Leadership: Taking Charge,” on Oct. 10.

-A new educational program will feature nine tracks based on the OMS scope of practice: anesthesia, cosmetic and orthognathic surgery, dental implants, dentoalveolar, head and neck oncology, pediatrics and cleft, reconstruction, temporomandibular joint and trauma. For each track, speakers will address the latest evidence-based research in a plenary session and then move into focused breakouts that will allow for greater discussion. In honor of the centennial, each track also will offer a historical review of the practice area and an oral abstract session.

Other notable events include:

A hands-on, full-day cadaver workshop covering the most common cosmetic surgical procedures of the facial region.

The Chalmers J. Lyons Memorial Lecture focusing on how induced pluripotent stem cells may be used for tissue regeneration.

Meet the Experts and Master Class sessions allowing attendees to discuss trends and innovations with legends in the specialty.

A new World Café, where senior surgeons will address patient cases and best practices while attendees will be able to connect with their peers from around the globe.

A series of team learning sessions, including an Overseas Surgery Team Program that will provide guidance to those interested in international service trips.

More than 30 practice management sessions to help improve the operations of OMS practices.

A two-day Beyond the Basics Coding Workshop that will review procedural coding, healthcare reform initiatives, audit tips and reimbursement issues.

ProSomnus® Sleep Technologies Product Platform Takes Aim at Key Oral Appliance Side Effects

Today, ProSomnus Sleep Technologies announces acceptance and presentation of evidence-based research on Oral Appliance Therapy as presented at the American Academy of Dental Sleep Medicine 27th Annual Meeting held June 1-3, 2018 in Baltimore, MD. ProSomnus supported research that resulted in study data demonstrating no significant tooth movement in 2.3 years.

The AADSM published a guidance paper on Management of Side Effects1 and called out “unwanted tooth movement” as the most common side effect in oral appliance therapy and calls for clinicians to “to minimize the side effects of OAT”. Norrhem, Nemeczek and Marklund2 reported the benefits of a rigid appliance over a flexible appliance in reducing tooth movement. The ProSomnus research further confirms the benefit of a rigid appliance to control the side effect of unwanted tooth movement.

The poster presented, “Tooth movement and bite changes for a hard-acrylic sleep appliance; 2-year results using the ProSomnus® MicrO2® Sleep Appliance”. Models and measurements were made independently at the Orthodontic department at the University of the Pacific.

• Used the Littles Irregularity Index to measure the impact of a hard-acrylic appliance on tooth movement over several years period of time.

• Reported results showed no significant tooth movement change over a 2.3-year period using the Littles Index.

• Addtionally, no statistical significant change was reported in overjet or overbite when the patient’s models were placed in MIP (Maximum Intercuspation).
