This article describes the use of temporary anchorage device (TAD)–assisted rapid palatal expanders in skeletally mature adolescents and young adults, who do not have the adaptative craniofacial growth that is characteristic of children and younger adolescents. Protocols for the use of TAD-supported virtually guided palate lateral wall expanders in both the nonsurgical and surgical management of transverse deficiencies are explained in this article.

This article discusses the limitations of aligners and dentoalveolar expansion in skeletally mature patients who require sutural expansion in addition to buccolingual tooth movement, and explains how TAD–supported 3D-printed palatal expanders, when used in conjunction with aligner therapy, can help avoid the need for bone augmentation periodontal surgery and allow for less invasive treatment in these patients.
